We’re sorry to see you go. Cancellations can be submitted at the club using the self-serve kiosk. 

If you’re unable to submit your cancellation in person, please mail a letter stating your name, date of birth, agreement number (if available), current address, contact phone number, and email address to P.O. Box 6800 Sherwood, AR 72124.
